Phone Service

Our phones are answered in the office from 8:30 am to 4:00 pm Monday through Thursday and 8:30 am to 3:00 pm on Friday.

Holiday Schedule

The office will be closed for all major holidays. At times during the year Dr. Glenn will be out of the office for seminars and a summer holiday. The office does remain open during these times with staff members here to answer questions and take messages.

Financial Policies

Please allow 24 hours notice when having to cancel an appointment. If no notice is given a $25.00 must be accessed to your account that will need to be paid prior to any additional appointments being made.

Emergency office visits will be charged a $25.00 Emergency work-in fee that will need to be paid prior to the appointment. This fee is not covered by insurance.

You are responsible for informing us of all insurances that you have and any changes to said insurance when they occur. Failure to do so will result in denied claims and all bills being sent to you. You will then be responsible for filing your claims with the correct insurance company.

Collection agencies are used in the event the responsible party refuses to pay the amount owed in an acceptable and timely manner.

We accept checks, however, we charge $42.00 per returned check. Once a check has been returned to our office we do not attempt a re-deposit. If the check is returned again your account will be charged the $42.00 and all future payments will have to be paid with cash, money order or credit card.

Prescription Refill Policy

Once you have received a prescription from our office, any request for a refill must be faxed to our office 3 business days prior to when the medication is needed.  After the 3 day period has passed please call your pharmacy to see if the medication is ready for you to pick-up.

If you require a hand written prescription for your refills you must call the office and leave a message on our prescription refill line with the request.  This line is only answered during our business hours.  Our answering service does not take prescription refill requests. The number to call is 269-4300 to request a hand written refill.  You will be routed to our refill line and will need the following information:

Your full name - please spell your name.  The name of the drug you are requesting.  Your phone number where you may be contacted if we have questions.

After the 3 day period has passed you may call the office to see if the written prescription is ready for you to pick-up.

Not all refill requests are granted.  You must allow 3 days for refills to be completed.  Do not wait until your last pill to call for a refill request.  There are times you may need to be seen by the physician prior to any refills.  If you fail to keep your appointment, we will not honor further requests for medication.  You are responsible for the safe keeping of your medications, keeping them out of reach of anyone other than for whom the medicine is prescribed.  We will not refill medications due to theft, loss, etc... without a police report.

Pain medications are prescribed short term, without refills.  We will refer you to pain management for persistent pain.

You are expected to get medications from one Doctor only.  If you do receive medications from other physicians you will need to immediately supply our office with a list of medications you are taking.  If you are receiving narcotics from another physician, as well as our office this may be grounds for termination of our services.

Medical Records Charge Policy

We charge a $1.00 per page for medical records copied up to the first 25 pages and .25 per page copied thereafter.

We will only release records with a HIPPA compliant release, which is fully executed and by our Privacy Policy and the policies set by the Secretary of Health and Human Services.  We charge for all costs that are incurred in the copying and sending of your records.  We will send one copy to your new physician, one time at no fee, if changing physicians.  All subsequent requests will be charged our regular fees.  These fees have to be paid in full prior to us releasing your records.  We have 10 business days to complete this process.

X-rays are not copied, they may be checked out but must be returned or a fee will be accessed to your account for the missing films.  These films are a permanent part of your office records and we are required by law to maintain them.  As of December 29, 2014, X-rays are available on digital disk for $15.00.

Medicare and Secondary Insurance

We will file claims with Medicare on your behalf.  If you have a secondary carrier to Medicare you will need to contact Medicare and give them all of your secondary insurance information so that they may submit your claims after they have processed them.  We will not file secondary coverage to Medicare.  If you do not have this service in place you will be held responsible for the balance left after Medicare pays their portion.  You may contact Medicare by using the phone number that is listed on the back of your card.
